- Application for this kind of dealership is subject for assessment and approval of Powerclean Chemicals’ Management.
- Accomplish the Dealer’s Information Form and submit together with the documentary requirements needed.
- Once approved, the dealer will receive an Authorized Dealer Certificate.
Required Documents:
- Photocopy of Business Name
- Photocopy of SEC registration for Corporation
- Photocopy of latest business permits (Mayor’s Permit)
- Photocopy of Identification Card (Driver’s license, SSS, etc.)
- Photocopy of latest Community Tax Certificate
- Sketch of location of Business Office and Residence
- Two (2) pcs. 2×2 latest colored ID picture
